Track and Field - Richardson, TX - Ages 6-12 / March 22- April 21 event registration

I consent for my child to participate on Runner2Runner running teams. I forever waive, release, and discharge any and all rights, claims for damages and causes of suits or action, known or unknown, that I may have against Runner2Runner team coaches, any sponsors, suppliers, agents, independent contractors, employees and any other persons who in any way assist or are connected with this team in any manner arising out of or resulting from my participation on a Runner2Runner team. I agree that this waiver, discharge and release also apply to my heirs, executors, administrators, and assigns without exception. I attest and verify that I have full knowledge of the risks involved and that I will assume and pay my own medical and emergency expenses in the event of an accident, illness, or other incapacity, regardless of whether I have authorized such expenses, and that I am physically fit and sufficiently trained to participate in running practice and events.

***I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing.

I further acknowledge that Runner2Runner LLC has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.

Masks are required (to be worn) for athletes and coaches

I further acknowledge that Runner2Runner LLC can not guarantee that my child (children) will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, coaching staff, and other Runner2Runner athletes and their families.

I voluntarily seek services provided by Runner2Runner LLC and acknowledge that I am increasing my child’s risk of exposure to the Coronavirus/COVID-19. I acknowledge that my child must comply with all set procedures to reduce the spread while attending Runner2Runner practices.

I attest that:

* My child (children) is not experiencing any symptoms of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.

* My child has not traveled internationally within the last 14 days.

* My child has not traveled to a highly impacted area within the United States of America in the last 14 days.

* I do not believe my child has been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.

* My child has not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non contagious by state or local public health authorities.

*My child is following all CDC recommended guidelines as much as possible and limiting his/her exposure to the Coronavirus/COVID-19.

I hereby release and agree to hold Runner2Runner LLC harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the salon, or that may otherwise arise in any way in connection with any services received from Runner2Runner LLC. I understand that this release discharges Runner2Runner LLC from any liability or claim that I, my heirs, or any personal representatives may have against the salon with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Runner2Runner LLC. This liability waiver and release extends to all Runner2Runner contractors, volunteers together with all owners, partners, and employees.

I have read and agree with Runner2Runner cancellation policy; 14 days prior to the first day of practice: $15 fee, 13 days prior to the first day of practice, 50% of registration fee refunded, once season begins there will be no refunds. If the season is shut down due to CDC/local restrictions, practices will be postponed and/or remaining practices will be issued in store credit for a future season.
